The Nitty Gritty:

• Limit of 2 vouchers per household
• Customer will need to sign an image release form confirming that they own the images to use for their canvas. Unlock My Story will not be able to produce any canvas if you do not sign this form.
• Canvas can only be shipped to locations in US
• No refunds if not used
• Once an order is placed & customers can contact info@unlockmystory.com. They will be sent an information sheet on how to submit their photos and family names (photos must be submitted electronically). A proof for the customer will be sent via e-mail and they will able to make one round of revisions before final approval. After approval on their canvas proof via e-mail, the canvas will be completed within 4 weeks. There must be 10 family members or less represented on the tree (otherwise the images and text become too small to see properly on the canvas).
• Must place order before 1/31/12
• Limit 1 voucher per transaction
• Entire voucher amount must be used in 1 transaction
• Voucher may not be combined with any other offers or promotions
• Expires 01/31/12
