• Appointments are required and subject to availability

• Cancellation/re-scheduling policy of 24 hours applies; voucher subject to forfeiture

• Does not include any dental work that may be needed prior to orthodontic treatment

• Exam is required to determine if you are a viable candidate for orthodontic treatment 
• If you are not a candidate your voucher may be transferred
